Flag State Inspection
1. Flag State Inspection (FSI) Initiation
- MTSI arranges FSI and informs the vessel.
- HSSEQ is kept informed.

2. Start of FSI
- Auditors board the vessel for inspection.
↓
3. Vessel Performs ISPS Checks
- ID verification.
- Bag check.
- Safety briefing.
- Emergency muster.
- Notify duty officer.
↓
4. Escort of Inspectors
- Senior officer escorts FSI officer to Master's office.
- Senior officer escorts FSI officer at all times during inspection.
↓
5. Major Non-Conformity
- If any major NC is found, contact DPA immediately.
- Liaise with MTSI to resolve any Major NC.
↓
6. Reporting to Office
- Primary: Create and attach FSI report via DSM reporting.
- Secondary: Send FSI report to MTSI with DPA in copy.
↓
7. Commence Rectification
- Liaise with agents.
- Liaise with vessel manager.
- Arrange for class as applicable.
↓
8. Capture Evidence of Close Out
- Take photographs.
- Gather training records.
- Collect statements.
- Obtain class survey reports.
↓
9. Closure of Deficiencies
- Master liaises with VM for RCA/corrective actions/preventive actions.
- Complete HSSEQ-01 form and include supporting documents.
