STOPER is an acronym for Stop, Think, Observe, Prevent and Execute. The STOPER card system is a behaviour-based safety observation program implemented onboard all Company vessels as a means to comply with the intervention policy.

It encourages all work team members to observe, identify, and intervene in any unsafe act and/or unsafe condition so that an incident can be avoided.

All officers and crew have the right and responsibility to stop a job and consult with their supervisor without fear of reprisal.

Simplified pocket versions of the STOPER Card are included in this appendix to enable every crew member to use a physical card and improve current safety measures.

Preparing the STOPER Card

  1. Soft versions of pocket-size STOPER cards (4) are included in this appendix.
  2. Heads of department shall ensure that colour copies are printed and laminated so that each crew member has one STOPER card.
  3. Once laminated, a transparent tape (magic tape) shall be pasted on fields which need to be filled out in pencil.
  4. Heads of department can print STOPER cards as needed—for example, if any card is lost or damaged or when extra crew members join the vessel.

Distribution of STOPER Card

  1. During the daily safety meeting, a STOPER card shall be issued to every crew member.
  2. Clear instructions shall be provided to each crew member on how and when to use the STOPER card.
  3. Each crew member shall carefully carry the STOPER card in their boiler suit pocket with a pencil to fill in data.

Actions Needed Once STOPER Card is Issued

  1. The crew member issuing the card shall verify that the work activity is SUSPENDED.
  2. The Head of Department or Officer of the Watch (OOW) shall be notified.
  3. Complete the card and submit it to the Head of Department.
  4. The Head of Department shall review the unsafe act/condition, take necessary risk mitigation measures, and make the environment safe for work prior to resuming the task. An alternate work method may be considered to achieve the objective of the task.
  5. Details of the STOPER card shall be updated in DSM > Near Miss or emailed to the HSSEQ department for records.
